Cooperative Work Experience Education (CWEE) Forms and Documents

Student Application - This triplicate form (sample document only - cannot be printed) must be signed and dated by the student/employee, the supervisor and the CWEE instructor.

Student Application Instructions - These printable instructions will assist you in filling out the Student Application correctly.

General/Vocational Training Agreement - This agreement (sample only - cannot be printed) is the most important and the most difficult to complete.  It requires that both the student/employee and the supervisor establish four training objectives which must be completed on the job during the semester.

General/Vocational Training Agreement Instructions and Learning Objectives Worksheet- These printable instructions will assist you in correctly filling out the Genera/Vocational Training Agreement.  The Learning Objectives worksheet needs to be submitted and approved by the instructor prior to turning in the agreement.  For more information, please refer to the course syllabus.

Employer's Evaluation and Verification of Hours - This form (sample only- cannot be printed) must be signed and dated by the student/employee and the supervisor and is due at the end of the semester.
